It seems that global play has been activated on the servers.
The 1.5.4 patch has no UI to change the servers within the game itself, yet.
But you can use
Simon's Relocalizer to change servers.

Yes.
In addition to that, you can publish maps in every region.

I think it was simply technical limitations: Blizzard apparently didn't want to spend the time/money to get b.net into a state where it was possible - IIRC there was no indication that it was ever going to happen in the future at the time of SC2's release. They basically added it as a feature for D3, and since they'd done all the work there, ported the changes back to SC2 now.

Another method to change the server has been found.
Put this xml file into your sc2 installation folder and you can see the region UI in the login screen.

To access that window via the button on the left side in the login screen, you need to go back until you can edit your e-mail address.
Publishing can be done globally now. We can change the server in the editor having that file.

Settings -> Battle.net -> select the region, save settings and publish it.
Yay... Only took them 3 years.
