Also, an addendum: Getting a level over an enemy (from a team push and 2-4 kills) is infinitely better than (at most) 300 gold from a creep wave.

Choosing when to farm. Holy shit if there was ever a time not to farm you guys have found some of these. If you're using spells with long cooldowns or large mana costs (Tort) to farm, you are lowering your effectiveness when the time comes to defend a lane. If you are not going to have the time to go to base and come back to a lane that needs you, you should not be farming. I have actually made 600+ gold from large creep waves due to Pyro's AoE spells. However for other heroes that do not have crowd control, or a melee lacking a Runed Axe, it definitely is even less worth it.

Runed Axe is so difficult with riki, since he doesn't auto attack.

Madman has to be the most item dependent fuck there is. With just one item, his power increases drastically. And each item makes getting all the other items so much easier. When I can't even build steamboots, I am fucked. I don't think you get that. I also think you underestimate how incredibly weak madman starts out. Actually he's always weak. He is built around getting up to the enemy unnoticed and start butt fucking them and before they can even react, half their hp is missing. He only survives long battles with super fast attacks with life leech.
In team fights, I suppose the best and only thing he can do is what I do, wait around the corner and once the battle initiates, come in from the back and start raping. He definitely can not initiate (except late, late game with uber items). And if the battle is over before it's begun, it really is pointless for me just to die. If our team is dissipating, throwing myself in the middle of 5 heroes will only be a fast death. Before you even have a chance to use it to some advantage, I'll already be dead.
If there's a possibility for me to cause any good, I do throw myself in the battle.
I know my limitations rather well.
When you purposely kill steal someone I'm raping and will definitely kill, you hurt me. It is sometimes too obvious my target will die and your spells are unnecessary. I've intentionally backed off the enemy when someone has dealt 90% of the damage and is going to kill him.
Really? Must be nice.
That is all I have to say
I might try different early item builds and see where I can find a successful early game. Maybe I go back to starting with boots to get in and out of the creep lane for last hits. Maybe a strength relic for extra hp. Maybe even runes of blight, though I try to go straight for hp regeneration for cost effectiveness.
I also think you should try different heroes, especially madman ;o

Madman's definitely the type that requires extensive babysitting to be used early effectively.
Have you tried a different hero? I find like the the ready out of the box type heroes like Night Hound (though he's, in my opinion, one of the more difficult heroes, tactic-wise). I think that you would like Blood Hunter a lot.
I gotta say that NH is prolly my favorite hero. He's not really that well understood by most people, dunno why, but you can use this to create confusion. Plus, placing smoke is an art form. Based on where they are in relation to creeps, you and their situation there are optimal placements. Say, for example, he's running, you want to get as far ahead of him as possible (about half a hero's length in front of your target) and cast it slightly in front of them so that as it unenvelopes, they just start walking into it. It's then that you start attacking from behind. They're faced with a decision. Go back towards NH and face possible further onslaught by others chasing or simply lose ground, or you can keep running forward into the smoke (which on an endgame speed takes about 2-4 seconds to traverse) getting a constant stream of nukes from NH's backstab (deals 1x agility in addition normal attack, essentially 100% crit when attacking from behind as you should gear for all agi vs dmg) all the while silenced.
My item build is almost always the same:
2x Slippers (+3agi)
1x HP Pot
1x Courier
- Game Starts
2x Circlet (+2 all stats)
2x Wraith Band recipe (+1 all stats +extra inventory space. +6agi,+3str/int total)
-- Send Courier to Hero to deliver items
1x Marchers
1x Steam Boots
3x total Wraith Bands
-- By this time, on a good game, I've just hit 6. On a bad game I'm usually halfway through completing my Wraith Bands. If this is the case, I often opt to get marchers before finishing the bands
1x Slash

Level 6 is when I start patrolling for ganks. My favorite is when an enemy hero's just finished a creep wave where they took heavy damage. I walk up (while invisible, of course) right behind them and cast cloud and start auto attacking. If t hey have an escape mechanism, they'll likely use it as soon as they're out of the cloud. I blink up to them immediately and usually finish them off by now. +200 gold and more than enough experience to make up for lost lane time (and usually I'm soaking up nearby laning exp too).

I don't know the Puppet Master's spells that well, but they're nice cc (crowd control) and damage.
With PP, you've got 2 disables. In addition, you've got your ult which puts down a bunch of attacking wards. And your chain lightning farming skill. Your primary item build will consist primarily of mana regen.
