Someone09 discovers a frag grenade in his rectum (inserted by a certain resident ninja who was, in truth, never killed by a pirate). He attempts to pull it out, but grabs it by the pin. The squeeze of his intestinal walls wasn't enough to hold the safety handle.

P.S.: O)Matt_Burch dies from making the first SeN v5 bad 'Fatal exception' pun.
