He senses something is wrong and he goes to check out what is happening. When he finds the stagecoach he notices the famous outlaw in his cowboy hat and his gang are robbing the stagecoach. They set off explosives and overturn the stagecoach. The outlaw sees the veteran watching him, but thinks there's no threat. The outlaw simply takes the veteran's horse and goes on his way.
On the other side of town a great Roman general is finishing up his campaign to conquer the Germanic barbarians. With the campaign over, the aging emperor goes to see the general and commend him on his accomplishment. The old emperor is dying and looking for a successor. He favors the general over his spoiled son.

The area around him is full of ash and rubble. The convict is collecting samples. When he finishes collecting the samples he goes back to the underground base where he lives. The scientists running the facility have called for him and have told him that he's to be sent on a mission to go back into the past. They offer him a full pardon if he goes back to the past and collects information on the virus that devastated the world.
A young boy goes to see the doctor and the doctor tells him his back is as crooked as a question mark and gives him leg braces. The boy has a very low IQ, but his mom is able to get him into public school. On his first day of school he meets a girl on the bus and they become friends. One day a group of kids is bullying the boy and throw rocks at him. The girl sees this and tells the boy to run. The boy does so, but the leg braces are in the way. As he's trying to run the leg braces fall off and the boy starts running as fast as lightning. His remarkable running ability gets the boy into college on a football scholarship. After he graduates he enlists into the army. There's a war going on in a far away country. In the army, he meets his new best friend who knows everything there is to know about catching and cooking shrimp. Together they are shipped off to war.
The outlaw is at the local town bar located 2 miles away from the site of the robbery and is celebrating when the local sheriff arrives and arrests him. In custody, the sheriff now looks to assemble a group of locals that will escort the outlaw to the 3:10 train that is going to the nearest prison. The old veteran volunteers to be part of the group that will escort the outlaw to the 3:10 train.
The emperor's son has found out about his father's wish. Enraged he goes to his father who is lying on his death bed. The public does not yet know about the emperor's decision to make the general his successor. In a fit of rage the son confronts his father and smothers him with a pillow. The following day the town discovers the emperor has died and think he has died of natural causes. They hold a great funeral in his honor.
The mission has started and the convict has been sent back in time. He finds that he's been sent back six years too early. He runs into some police and gets put into a mental institution based on the diagnosis of a woman. In the mental institution he meets a crazy animal rights activist. The two talk and the convict tells the activist that there's a virus that's going to wipe out humanity. In the future, the scientists correct their error and send the convict back to their time, but not before revealing that they have found an old voicemail from the past that tells that the group known as the Army of the Twelve Monkeys were responsible for the virus.
The boy, now a young man in the army who has been shipped off to a foreign country now meets his lieutenant. It seems like everyday the platoon is just walking through the jungle and through the rain. The young man doesn't really understand why the war is happening, but he knows that it's the foreign country's civil conflict that the town is now involved in.
The local group and the convicted outlaw are making their way to the train. Along the way they encounter many troubles and several members of the group die, but the veteran is still alive. Finally it's just down to the outlaw and the veteran. The veteran confesses to the outlaw that he lost his foot in the war because he was shot by fellow soldiers when they were retreating and that he's ashamed to tell his son the story. He's volunteered to be part of this group because he thinks it will be a great accomplishment to escort the outlaw to the station which he can tell his son about. The outlaw agrees to let the veteran take him to the 3:10 train.
The emperor has ordered for the death of the general and his family. The general is captured by a group of Roman soldiers and is taking into the woods to be executed. However, the general manages to take one soldier's sword and kill all his captors and escape. Now free, he rushes to his family. When he arrives home he sees the burned and hanged bodies of his wife and young son. Overcome with grief, the general collapses to the ground. Soon he is discovered and captured by a traveling band of slave traders. The former general is taken to the market where he is sold to the owner of a gladiator school. After initially refuses to fight, the general turned gladiator starts to make a name for himself as the "Spaniard". He finds a masked helmet and uses it to conceal his face. The emperor's son who has now taken the role of emperor decides to pay a tribute to his father and also gain the favor of the people by hosting gladiatorial games at the Colosseum. The group the gladiator is a part of has been chosen to fight in the games and the Spaniard sees this as an opportunity to get close to the emperor and take his revenge. When the gladiators arrive at the Colosseum they realize that they are part of a reenactment of the Battle of Zama, a battle which they are historically supposed to lose. With the odds stacked completely against them, the Spaniard is able to organize his fellow gladiators and with his leadership skills he is able to lead them to victory. The surprised emperor goes to meet the Spaniard after the battle and congratulate him. He orders the Spaniard to remove his helmet, and when the Spaniard does so, the emperor sees that it is the former general. The general vows to the emperor that he will get his revenge. The emperor challenges the Spaniard to a duel.
The convict has now been sent back in time again, this time to the right year. Now in the past, the convict has kidnapped the same woman who diagnosed him six years earlier. He also learns that the man he met in the mental institution is the founder of the 12 Monkeys. The convict goes out in search of the man, and when he finds him the activist denies having anything to do with the virus. The woman eventually comes to realize that what the convict is saying is true and that he isn't crazy. She then leaves a voicemail on the number the convict has given her which talks about the 12 Monkeys and the virus.

He does this several times as each time he finds a member of his platoon, but not his friend. Eventually he stumbles onto his lieutenant who has been gravely injured by an explosion. The lieutenant tells the young man to leave him and let him die on the battlefield as there has been a member of his family who has died in each of the major wars in history. However, the young man refuses to abandon the lieutenant and let him die so he takes him on his back and runs him to safety. The young man goes back into the jungle one final time to find his friend who is still missing.
The outlaw and the veteran have made it to the station, however they see that the outlaw's gang has set themselves up around the station and are waiting for the outlaw. The outlaw still wants to board the train and honor his agreement with the veteran. The veteran's son who has been following them creates a diversion by letting loose a stampede of cattle. This catches the gang off guard and allows the outlaw to board the train.
Before the duel, the Spaniard is chained up and the emperor goes to meet him. The emperor walks up to the Spaniard and stabs him in the ribs with a knife to weaken him before the fight. He then orders his servants to conceal the wound, putting the Spaniard's armor on.
The convict and the woman have decided to go to the airport and run away together to enjoy the time they have left. On the way they hear on the news that the 12 Monkeys have let loose a group of animals. The two are relieved that the 12 Monkeys was just a red herring and that they were harmless.
The young man is in the jungle and finally stumbles upon his friend. When he looks at his friend he sees that there's a piece of wood stuck into his gut. The young man tells his friend he won't leave him out here and so he picks him up and runs him to the safety of the beach. However, his friend's injuries are too severe.
EzTerix dies in the young man's arms a victim of war killed by that foreign country.
The outlaw congratulates the veteran on his accomplishment, but also mentions that he's been to the same prison three times and has escaped each time. Then the outlaw's second in command shows up and raises his gun to the veteran. The outlaw orders him to stop but the second in command ignores the order and shoots the veteran 4 times. The second in command tosses his boss his gun belt. After a moment of silence, the outlaw looks at
FaRTy1billion and executes him.
The emperor and wounded Spaniard enter the ring and the duel begins. The emperor starts off with the upperhand and he knocks the Spaniard's sword out of his weakened grasp, but the Spaniard summons the strength to fight back and starts to pummel the emperor with his fists. The emperor draws his dagger and tries to stab the Spaniard, but the Spaniard catches his arm and turns it back on the emperor. A struggle ensues as the Spaniard overpowers the emperor like a demon possessed and forces the emperor to drive the dagger into his own heart killing him.
BiOAtK, with a dagger in his heart, falls to the ground and dies. The exhausted Spaniard collapses to the ground and sees visions of his wife and son in the Elysium fields with him.
The convict and the woman are now at the airport waiting on the security line. Ahead of them they notice a scientist who worked for the company that produces the virus and they overhear his conversation with the security man saying how he's traveling to several places around the world. The convict realizes that the list of locations matches the pattern of outbreaks that occurred when the virus hits. With the knowledge that the scientist is the one who is going to release the virus, he draws out his gun and runs to stop him. Security sees him doing this and one guard dressed in all black takes out his gun and with a look of joy on his face shoots the convict before the he can make it to the scientist. As the convict falls to the ground he notices a young child looking back at him. As
Changeling dies in the woman's arms, he realizes for the first time that his recurring dreams of seeing a man being shot in the airport are memories he's seen as a child.
The young man has made it safely out of the jungle and is now resting in the hospital. In the hospital he meets his lieutenant who has survived thanks to the work of the doctors although he has lost his two legs.
The veteran's son watched the whole thing and now has his gun pointed at the outlaw. The boy is so close to the outlaw that he can see his cold blue eyes and the specks of dirt on his shirt, but he finds that he can't pull the trigger. Instead, he goes to his dying father's aid. In his son's arms,
Tempz dies. The outlaw boards the train and as it takes off he whistles. In the distance, his horse hears the whistle and takes off towards the train.
